When Tom first came to Achora in October 2023, he was in a precarious position. Having recently transitioned from another provider, he brought with him a nearly depleted NDIS plan, limited access to supports, and growing anxiety about what would happen next. The lack of coordination had left him feeling isolated, unsupported, and fearful that everything he had worked toward might unravel.
Tom lives with major anxiety and depression, compounded by past trauma. Even the smallest disruption to his routine could lead to a significant decline in his mental health. So when it became clear that his funding was nearly exhausted we knew action had to be taken fast. The potential consequences were serious. Tom could lose access to social connection, mental health supports, and structure leading to a rapid deterioration in his wellbeing.
But Tom wasn’t alone anymore. That’s where Achora stepped in. We’ve seen it too many times before. NDIS providers that make an already complex system seem unfair or more confusing. But the NDIS is built to support participants, and the right companions can make it happen.
NDIS Support Coordination in Action: Restoring Stability and Access to Services
Our support coordination team immediately took action and made sure Tom’s NDIS plan was working for him. We reviewed his NDIS portal daily, connected with service providers, engaged the Support Approver to release funding where appropriate, and ensured essential services were sustained. At the heart of this effort was one goal: protect Tom’s stability while preparing for his upcoming plan review.
By ensuring continued access and funding to group activities, Tom remained socially connected, which is an essential protective factor for someone managing anxiety. We worked to maintain his involvement in cooking groups, personal skills development, and community routines. These weren’t just services, they are his lifeline and his right to be included in the community.
The plan review went ahead as scheduled and resulted in a positive outcome. Tom finally had room to breathe again, with renewed funding, new goals, and a stronger support system in place.

But something even more powerful emerged during this time.
During one of our regular visits, Tom handed his support worker a typed essay he’d been working on. It was a reflective piece on economics, diving deep into the disconnection between financial systems and the natural world. Written in his own voice, coming entirely from his own research and perspective, it offered raw and thoughtful insights grounded in personal experience.
As a former financial services professional, Tom had long harboured a deep interest in systemic issues. This was his way of reclaiming that part of himself and expressing his thoughts through writing. When our team read Tom’s writing, not only were we so impressed, but incredibly proud of Tom. It was more than just an article. It was Tom reclaiming his voice.
On our next visit, we shared our thoughts. We told him how moved we were by his work. His response?
“I’m over the moon! I’m so happy that my writing impressed you.” And expressed a renewed motivation to write a documentary and share more of his voice with the world.
This was more than a moment of praise. It was a turning point. Tom had rediscovered purpose through creativity, and that spark came from feeling supported, encouraged, and heard.
Building Daily Living Skills With NDIS Core Supports
Today, Tom is flourishing in his own way.
He continues to engage with community activities that keep him socially connected and emotionally regulated. With the help of his support workers and community programs, he is learning to:
- Cook his own meals
- Manage his personal budget
- Maintain his living environment
- Build consistent daily routines
These are all part of his NDIS core supports, which are designed to build capacity and reduce reliance on formal care over time. While he continues to manage anxiety and the effects of trauma, Tom is now doing so from a place of stability and self-belief.
And behind that growth is consistent, compassionate, and responsive coordination. Achora has remained his companion through his NDIS journey, making sure that every available resource is aligned with his goals. And, advocating for him when his voice needed to be heard most.
