Navigate your NDIS Plan with Achora

With over 8 years of experience as a trusted NDIS disability provider, Achora has successfully supported hundreds of clients from diverse backgrounds. Our support coordination service is built around real people with real goals—not just paperwork. Between understanding your funding, knowing what you can actually use it for, and trying to organise support that actually works for you, it’s easy to feel overwhelmed. That’s where we come in.

We focus on clear, personalised communication to ensure you fully understand your options and entitlements. Our goal is to simplify the NDIS system, tackle challenges together, and help you access services that align with your unique needs and aspirations. With Achora, your journey is tailored to empower you and achieve your personal goals.

We know that support coordination is not a one-size-fits-all solution. We prioritise taking the time to get to know each and every participant. We listen to what’s working (and what isn’t), and then help you build a plan that fits your life and feels right for you.

Why Choose Achora for NDIS Support Coordination

Choosing the right Support Coordinator can make all the difference in how you experience the NDIS. At Achora, we don’t just help you “use your plan” we help you understand it, feel confident in it, and build supports around your life, not the system. Our NDIS Support Coordination services are built to put you in the driver’s seat, with a trusted team walking beside you the whole way.

We help you find and connect with service providers that genuinely suit your needs. Whether they’re community supports, therapists, informal carers, or funded services. If things change, no problem. You can switch coordinators or adapt your support at any time. Whether you’re in a major city or a remote community, we’re here to make sure language, location, or complexity never stand in the way of getting the support you deserve. With Achora, support coordination is simple, flexible, and made for you.

Understanding the Different Levels of NDIS Support Coordination

Support Coordination Levels

The NDIS offers three levels of Support Coordination, each designed to match the different needs, goals, and challenges of participants. Whether you’re looking for basic guidance or complex case management, we’ll work closely with you and your support network to find the right level and make sure you’re getting the most out of your NDIS plan funding.

Support Connection Or Recovery Coaching (Level 1)

This entry-level support is often called NDIS Recovery Coaching when related to mental health and focuses on helping participants build the skills to connect with both formal and informal supports. At this level, we’ll work with you to:

  • Understand your NDIS plan
  • Identify the services you can access
  • Connect you to local providers, community resources, and informal supports like family or friends
  • Build confidence in managing your own plan over time

It’s ideal for participants who can manage some elements of their plan but need a little guidance to get started.

Support Coordination (Level 2)

Designed for participants who require regular assistance to manage and coordinate their supports, this is the most common level of support. As your NDIS Support Coordinator, Achora will:

  • Help you understand your plan goals, funding categories, and entitlements
  • Find the right mix of providers for your unique needs
  • Organise and schedule your supports
  • Address and solve issues and advocate on your behalf if services aren’t working
  • Help you prepare for plan reviews and adjust supports as your goals evolve

This level is great if you’re looking for a steady partner to help you navigate the NDIS, stay on top of your funding, and empower you to make confident decisions.

Specialist Support Coordination (Level 3)

Participants with high or complex needs, such as those with multiple service providers, crisis situations, or barriers to accessing consistent supports will benefit from this level. Your Specialist Support Coordinator at Achora will:

  • Provide intensive, expert-level support
  • Manage and coordinate complex service arrangements
  • Respond to high-risk situations or urgent changes
  • Work collaboratively with government services (like justice, housing, health)
  • Develop long-term strategies to stabilise your support environment

We specialise in high-intensity support coordination and understand how important it is to have someone you trust managing complex systems and services on your behalf.

Not sure which level you’re funded for? Get in touch, and we’ll help you understand your plan and match you with the right level of support coordination.

Am I Eligible for NDIS Support Coordination?

Not everyone who has an NDIS plan will automatically receive Support Coordination funding—but if you’re facing certain challenges, you may qualify for this important service. Eligibility for NDIS Support Coordination is based on your individual circumstances and the level of support you need to manage your plan effectively.

You may be eligible for Support Coordination if you:

  • Have complex support needs involving multiple providers or services
  • Experience psychosocial disability, mental health conditions, or fluctuating capacity
  • Have limited informal supports (e.g. no family or friends to help)
  • Face communication difficulties or barriers to understanding your plan
  • Are socially isolated or lack the confidence to engage with providers
  • Struggle to make informed decisions about your supports or goals
  • Are new to the NDIS and need help getting started

If any of these apply to you, it’s important to raise your concerns during your NDIS planning meeting or plan review. The NDIA may include Support Coordination in your plan if they determine it’s reasonable and necessary to help you reach your goals and manage your supports.

Our experienced team at Achora is here to support you. We can help you prepare for your planning meeting, explain what to ask for, and provide guidance if you think you need Support Coordination but aren’t sure where to begin.

Reach out today, and we’ll walk you through it—step by step.

  • Support your decision-making, helping you stay in charge of your plan while giving you the information and guidance you need to make informed choices
  • Connect you with trusted service providers, from therapy to housing to community programs, based on what suits your needs best
  • Negotiate services and pricing on your behalf to ensure fair value and alignment with your budget
  • Coordinate all your supports, making sure your services are working together smoothly
  • Help you manage your NDIS budget, so you stay on track with funding and avoid under- or overspending
  • Organise assessments and reports, including any required documentation for therapies, supports, or assistive technology
  • Prepare for NDIS plan reviews, helping you reflect on what’s worked, adjust your goals, and request any necessary changes

Whether you’re new to the NDIS or adjusting your current plan, our goal is to simplify the system, reduce your stress, and make sure your supports genuinely work for you.

Frequently Asked Questions About Support Coordination

If your NDIS plan feels overwhelming or you’re unsure how to connect with providers, you may benefit from Support Coordination. It’s especially helpful if you have complex needs, limited informal supports, or are new to the NDIS and need help navigating the system.

Support Coordination is not automatically included in every plan. You’ll need to discuss your needs during your initial planning meeting or at your next plan review. If you can show that you have complex circumstances or need help managing your supports, the NDIA may fund it as a “reasonable and necessary” support.

Yes. If you’re plan-managed or self-managed, you can choose any support coordinator—including non-registered providers. If you’re NDIA-managed, you’ll need to choose from registered NDIS providers like Achora.

Absolutely. It’s your plan and your choice. If your current support coordinator isn’t meeting your needs, you can switch to a provider who’s a better fit.

Yes. One of the key roles of a support coordinator is helping you prepare for your NDIS plan review. This includes reviewing what’s worked in your current plan, gathering provider reports, updating your goals, and helping you advocate for changes or additional funding if needed.

It’s entirely up to you. Depending on the level of support coordination included in your plan and your personal needs, you can let your support coordinator know how often you’d like to hear from them. Some participants need regular weekly or fortnightly contact, while others may only need occasional check-ins. At Achora, we tailor our communication to suit you—whether that’s frequent support or just being available when you need us.
